12 RC 3 Assessment of volume responsiveness

چکیده
Assessing hypovolaemia and predicting volume responsiveness is a difficult challenge in most critically ill patients and surgical patients. Clinicians have to deal with three different scenarios: • Patients who present in the Emergency Department with evident acute body fluid losses. The diagnosis of hypovolaemia is almost certain and a positive haemodynamic response to volume resuscitation will occur. Patients admitted to the Emergency Department likely to be in septic shock. There is no need for sophisticated parameters to predict volume responsiveness since a positive haemodynamic response is always expected. • Patients who have been in the ICU for several hours or patients in the operating room who experience haemodynamic instability. Fluid administration may represent a therapeutic dilemma. On one hand, one may expect a beneficial effect of fluid administration if the heart still has some preload reserve. On the other hand, because the patient has often been resuscitated already, the presence of preload reserve is not guaranteed and further fluid infusion has the potential to cause harm through development of pulmonary oedema.

Influence of Real Ground Motion Records in Performance Assessment of RC Buildings
Reinforced concrete frame buildings with Open Ground Story (OGS) are one of the most common building configurations in urban habitat. These configurations are known to be vulnerable to seismic excitations, primarily due to the sudden loss in strength in the ground story and differential stiffness distribution throughout the structure. The differential stiffness distribution is attributed primar...

Background and Objectives: Responsiveness is one of the hallmarks of high performance health systems. Maintaining the responsiveness of health organizations at high level, require constant assessment of its situation as perceived by the patients. The accumulation of data on patients’ perception of health organizations’ responsiveness can help policy-makers in developing effective relevant strat...
متن کاملStudy of Variation of the J-integral and the Fracture Toughness in Blunt V-notches under Mode I Loading
Fracture assessment of U- and V-notches is important in mechanical engineering. One can use the J-integral as fracture parameter in order to predict the critical fracture load in notches. The critical value of the J-integral in cracks is a function of the material properties. In notches, however, the material properties as well as the notch dimensions affect this critical value (named fracture ...

متن کاملSeismic Damage Assessment of RC Buildings Subjected to the Rotational Ground Motion Records Considering Soil-Structure Interaction
The significance of the seismic rotational components have been overlooked in the seismic evaluation of structural behavior. As researchers have measured seismic components more accurately using sensitive rotational velocity sensor, it was observed that the magnitude of rotational components is considerable and could not be neglected. Hence, some parts of seismic damage or failure of structures...
